When AAA ultrasound is covered

The U.S. Preventive Services Task Force recommends a one-time ultrasound screening for abdominal aortic aneurysm in men ages 65 to 75 who have ever smoked. Medicare defines eligibility as a family history of AAA or a man ages 65 to 75 who has smoked at least 100 cigarettes in his lifetime, and requires a referral.

For an eligible Medicare beneficiary, the screening is covered once in a lifetime with no payment when the provider accepts assignment. Private plans use their own network and benefit rules, so verify those separately.

Men 65 to 75 who never smoked may be offered screening selectively, often based on family history or other risk factors. The key point is that this is a one-time screening event for most eligible men, not an annual test.

When it is not routine

If the ultrasound is ordered because of abdominal pain, a pulsatile mass, or another concerning finding, it is diagnostic rather than preventive. In that situation, coverage follows your plan's diagnostic imaging rules.

What this does not mean

A covered AAA ultrasound does not mean every man needs one. It also does not screen for all vascular disease. The test looks specifically at the abdominal aorta and is a one-time event for most eligible men. It will not detect blockages in the leg arteries, carotid disease, or coronary artery disease.

How to confirm coverage

Ask your clinician whether you meet the age and smoking criteria. Then verify with your insurer that the imaging center is in network and that the order will be submitted as preventive AAA screening. Confirm whether any follow-up scans would be covered if the result is abnormal.

If you have a family history of aneurysm, ask whether that changes your eligibility and how the test should be coded. Some plans may cover earlier or more frequent imaging when a strong family history is documented.
