The 40 shift · Screenings by age

What Men Should Get Checked at 40

What actually changes at 40 for healthy men: blood pressure, diabetes risk, family history, and a few tests to skip.

The direct answer

At 40, blood pressure checks become more important, diabetes screening is appropriate if you carry extra weight, and it is time to take family history seriously. Most men still do not need stress tests, calcium scans, or whole-body MRI just because they turned 40.

Blood pressure and metabolic screening

By 40, yearly blood pressure screening is reasonable for most men. If a reading is elevated or you have risk factors such as obesity, sleep apnea, kidney disease, or a strong family history, your clinician may want checks more often.

Diabetes screening is recommended for adults ages 35 to 70 with overweight or obesity. This is also the age to review cholesterol, triglycerides, and lifestyle patterns. Heart disease is the leading cause of death for men in the United States, and these basics matter more than a dramatic scan.

Cancer context before 45

Colorectal cancer screening starts at 45 for average-risk adults, but your family history could move that earlier. Ask about relatives with colorectal cancer, advanced polyps, or certain inherited syndromes. Prostate-specific antigen testing is not a routine requirement at 40; the meaningful PSA conversation usually happens later.

  • Average-risk colorectal screening begins at 45.
  • A family history of early colorectal cancer may shift the start age.
  • PSA is not a standard test at 40 for most men.

What this does not mean

Turning 40 is not a license to order coronary calcium scans, routine ECGs, stress tests, or whole-body MRI. For asymptomatic low-risk adults, screening ECG is not recommended, and calcium scoring is a selective discussion, not a default birthday present.

Feeling well is reassuring, but it does not replace the basics. A normal scan does not erase the benefit of blood pressure control, not smoking, and moving your body. Sources checked July 20, 2026. Independent clinician review is pending.

Questions to bring to a visit

Come with a blood pressure trend if you have home readings. Note your family history of cancer and heart disease, your smoking history, and any new symptoms. Ask what screenings are due now, what waits until 45, and whether any recommended test will be billed as preventive.

  • What is my blood pressure trend, and how often should it be checked?
  • Does my family history change when I should start colorectal screening?
  • Will this test be coded as preventive under my plan?

Frequently asked questions

Do I need a colonoscopy at 40?

For average-risk men, no. Routine colorectal screening starts at 45. It may start earlier if you have a strong family history or certain genetic conditions.

Is a coronary calcium scan needed at 40?

Not routinely. Calcium scoring may help selected men after a risk discussion, but it is not recommended for all asymptomatic low-risk adults.

How often should blood pressure be checked at 40?

Yearly is reasonable for most men. If it is elevated or you have other risk factors, your clinician may recommend more frequent checks.
